Please fill out the following information and include it with your entries in Team Manager Format CL2, your payment (one check per team made out to Wooster Swim Club Boosters is preferable) and your t-shirt order form along with t-shirt payment.  If for some reason you cannot return your entries in Team Manager Format CL2, please include a $2.00 per swimmer processing fee along with the paper entries.

SEA SPRITE & OPEN ENTRY FORM

Dear Parents:

If your child is planning on swimming in the 16^th annual Sea Sprite & Open Meet, please fill out this entry form and return it to Coach                         no later than December 19, 2008.  Please use a separate form for each swimmer.  Payment should be included with the form(s).  The fee for Sea Sprite is $3.00 per event, swimmers may enter up to 4 events total.

T-shirts for Sea Sprite are being sold on a pre-sale basis.  They are $15.00 each.  The shirts will be royal blue with white lettering and will depict playful aquatic monsters competing for the King of the Water title in the “2009 Sea Sprite Battle of the Titans”.
